What happens to the picture
The video track, any subtitles and chapter markers are all discarded. Only the audio track survives, and the length of the resulting MP3 matches the video's audio exactly, whatever the video's resolution or frame rate was.
Why MP4 audio isn't already MP3
Most MP4 videos hold AAC audio, a different lossy codec from MP3, not raw MP3 data. AAC has been the default for cameras, phones and streaming video since the early 2000s, largely because it sounds better than MP3 at the same bitrate. Extracting it to MP3 re-encodes AAC into MP3, which is a second lossy step rather than a lossless copy. For most videos the difference is not noticeable, but pick 256 or 320 kbps if the source audio quality matters to you.
This is also why the file you get back is never bit-for-bit the same as some separately downloaded MP3 of the same song or speech might be. It's a fresh encode built from your specific video's audio track, not a lookup against any other copy.

Does converting to MP3 keep the video?
No. Only the audio track is kept. The picture, any subtitles and chapter markers are discarded, so the MP3 has no video in it.
What bitrate should I pick?
192 kbps sounds close to the original for most music and speech. Use 320 kbps for the best quality, or 128 kbps for lectures and podcasts where a small file matters more.
Can I convert a video that has no audio?
No. If the video has no audio track, there is nothing to extract, and you will see an error.
How big will the MP3 be?
Roughly the bitrate times the length. A 5 minute video at 192 kbps comes out around 7 MB, and the same video at 128 kbps is closer to 5 MB.
Does the video's resolution affect the MP3?
No. Resolution only affects the picture, which is thrown away. Only the length of the audio track and the bitrate you pick affect the MP3's size and quality.
